PC.13. The ACS program conduct monthly and as needed mortality and morbidity meetings.
PC.13.1. All mortality and morbidity cases are reviewed on ongoing basis to highlight avoidable and potentially avoidable cases.
PC.13.2. Morbidity cases are captured from program indicators monitoring and from reports from procedural areas.
PC.13.3. Highlighted cases are distributed among peers before the meeting to identify areas for improvement.
PC.13.4. Shared cases between departments are referred to the medical director to decide on presenting it to the hospital morbidity and mortality committee.
